Click on the Patients tab in the top menu, select your patient from the list and click on the Care Plans tab in the left side menu that appears.
The main Care Plans screen will show the care plans listed in a table. You can choose to show only the current ones, archived or all care plans by using the view toggle drop down list in the toolbar at the top (Figure 1).

To add a new care plan, click the Add New Care Plan button and simply select the approprate care plan from the list provided by clicking on it. You can browse through the list alphabetically by using the page numbers at the bottom, or alternatively use the Search feature at the top to find the care plan you are looking for (Figure 2). You can also filter the list of care plans to those that are relevant to your patient using the ethnicity, gender and age drop down list options.

The schedule for the care plan you have selected will then appear (an example shown in Figure 3). If this is right, click the Choose This Care Plan For Patient button. If this does not appear to be the care plan you were looking for, click the Back button and try another.

The new care plan will now appear in the list back on the main page. To view te patient's schedule in detail, click the View All Activities button. You can change the time frame in which you view the schedule by using the drop down list options in the toolbar. You can also check on any activities scheduled for the current day by clicking the Today's Activities button (Figure 4).

To enter details of an activity in the schedule, click on the square for that item and then the date link in the information that pops up (Figure 5). Squares marked for activities will appear red if missed, blue if upcoming and green once completed.

Enter the actual date scheduled for the event, once this is confirmed, using the calendar button, or use the Use Proposed Date or Set To Today buttons if relevant. Select the person who the activity was/will be administered by, enter the location and, if relevant, the results form the assessment. Then enter any comments (Figure 6). To save the details, click the Add button. Alternatively to cancel, click the Cancel button. Once the details are saved, the square for that item should turn green.
