MMEx Care Plans
MMEx's patient management system also allows you to create, manage and share care plans for patients. A variety of care plans have been designed for the management of specific diseases such as, diabetes, asthma, and heart disease.
Each care plan has a number of activities associated with it, and MMEx provides a calendar that allows you to view a care plan from the perspective of 1 month to 20 years. In Figure 1 below, a two year view of a Hepatitis C Carrier care plan can be seen.

To add a new care plan, click the Add New Care Plan button (Figure 2).

You can then use the search bar and page numbers to browse the care plans available. Notice the filters in the toolbar next to the search bar. These are used to filter those care plans only relevant to the patient.

Figure 4 shows a preview of the list of activities for the Hepatitis C Carrier care plan which was selected from the list (shown above in Figure 3).

Click the Choose This Care Plan for Patient button to add the care plan to the patient record.

Click on the Schedule button. To view and edit the information of a particular activity, click on the square for that activity in the calendar grid and click on the date link in the box that pops up.

You can then record results from the appointment for each activity in the care plan by completing the fields in the box that appears.

To save the information, click the Add button.
